Telma Product Return Form
All warranty product must be returned to Telma. Use the following document as a packing slip when returing product to Telma. (Please write your Incident ID# in bold marker on the outside of the shipping box!)

Installation Measurement Checklist
For mechanical problems with the retarder, a measurement checklist will need to be submitted to Telma. This provides the driveline angles and measurements to verify proper installation. (Physical measurements are not needed for electrical problems)
